                  Candidate Submittal

                 Candidate Requirements:

            

1. All candidates must hold the rank of Black Belt,  1st Dan or Above      and be in attendance at the Hall of Fame Banquet.

 

2. Candidates must be between the ages of 12 to 22 years, both inclusive, and a Student in elementary, high school, Jr. College or a University.

 

3. They must have achieved high academic standing (B average or higher), attained competitive status at the state or national levels, participated in community service activity, and demonstrated the personal attributes of Chris such as motivation, humility, perseverance, care and concern for others, sportsmanship, etc.

 

4. An individual may be recommended by a parent, friend, coach, instructor, Relative, etc. Those making the recommendation must be at least 21yrs. old.

 

Submittal and Selection Process

 

1. All submittals must be type and contain the information to support the accomplishment in all four standards of the award-academics, martial arts accomplishments, community service and demonstrated personal attributes.

 

2. All submittals must be mailed, with the applicant's Resume to:

GMAHOF, 12 Park Ave. St Thomas, N5R4V3, Ontario, Canada.

 

By July 1, of the current year. Any submittals for the scholarship received after that date will not be considered.

 

3. Any misrepresentation or false information will result in the submittal being Disqualified and withdrawn.

4. The person submitting the candidate should give their contact information.  Should some additional information be required or a clarification is needed On what was submitted.

5. Once the deadline of July 1st.  has passed, the selection committee will review all submittals. The candidate who best meets the requirements of the award and most represents what Chris Canning stood for will be determined the Winner.

 

6. The selection committee's decision is final and no appeal process      is Provided.

 

                              Suggested Submittal Format

     1. Submitter information-name, address, phone, email, relation to the Candidate, martial arts school affiliation, location, their address and phone.

2. Information on the candidate-name, age and birth date, rank, date achieved, school attending for education, martial arts school, how many years of training in the martial arts. In addition, information on the four standards:


 

a) What is the candidate's grade point average, standing in the class, and

          What grade is the candidate presently in?

b) His or Her competitive martial arts experience-How many local, regional, State, National and Junior Olympic tournaments have they competed in. What titles have they won? How many total placements have they won in forms, sparring, breaking, etc. Their highest achievement. What training have they had?

 

c) Community service or involvement.  Name the projects or organization they are a member of, or what work they did or are now doing.

     d) Personal attributes-describe how they have demonstrated humility, Perseverance, concern for others, sportsmanship, etc

3. What was their toughest competition and why you considered this hardest?

4. Any additional references, comments or outstanding achievements or Recognitions.
